OPTIONS FOR REGISTERING
Registration for events is on a first come first serve basis

1. Fill out Registration Form (BELOW) and email it to ccmr@mtu.edu
Call 906-487-2572 with payment information AND FAX (906-487-3027) or mail in the first page of the registration form with signed liability form.

2. Complete and print out Registration Form.
FAX the completed form to 906-487-3027 or Mail it to CCMR, Seaman Mineral Museum, Michigan Tech, Houghton, MI 49931.

3. Request hard copy of Registration Form by calling 906-487-2572
Mail it to CCMR, Seaman Mineral Museum, Michigan Tech, Houghton, MI 49931.

4. Phone registration by calling 906-487-2572 and DO NOT leave a message.
FAX (906-487-3027) or mail in the first page of the registration form with signed liability form.

YOUR REGISTRATION WILL BE CANCELLED IF WE DO NOT PROMPTLY RECEIVE PAYMENT OR COMPLETED LIABILITY FORM

Parents need to complete a Parental Consent form for all Children
